Regional Director, Gavin Ward, Recognized as 1st Certified Fellow in Readmission Prevention
The National Readmission Prevention Collaborative Summit 2014 was held in Anaheim on May 7th, 2014. The goal of the conference was to bring together leaders in the fight against hospital readmissions and to share best practices. Many readmission prevention leaders from throughout California and the nation came together including leaders at the highest levels of hospital organizations and post-acute providers.
24 Hour Home Care’s Southern California Regional Director, Gavin Ward, was recognized at the conference as the 1st Certified Fellow in Readmission Prevention by the National Readmission Prevention Collaborative. The fellowship included significant research, studying, and calls to action in providing solutions to the problem of readmissions from a post-acute provider’s perspective. Over 500 leaders in healthcare attended the conference.
