The American Case Management Association- Northern California Chapter will host the 10th Annual Case Management Conference at the Meritage Resort & Spa in beautiful Napa, California on June 30th.

This year’s conference theme is, “Building Your Medical Neighborhood: Partnering For The Future Around the Affordable Care Act.” The day-long conference will feature workshops on topics such as how to create a model care transitions program, how to build new community relationships to ensure safe care transitions, and public policy changes.
