24 Hour Home Care Sponsors the First Annual Northern California Readmission Summit 2014
24 Hour Home Care had the privilege of supporting the Northern California Readmissions Summit in San Francisco in September, including the opportunity to moderate a panel of thought leaders from Kaiser Permanente, Stanford Health Care, and Sutter Health. Healthcare leaders from Northern California were present to collaborate on preventing hospitalizations. The crowd had the privilege of hearing Dr. Eric Coleman, creator of the Care Transitions Initiative™, one of the most popular care transitions models across the country, share best practices regarding how to encourage patients to engage in their own recovery and well-being after leaving the hospital setting.
