Patrick Soon-Shiong Innovation Awards Selects 24 Hour Home Care as a Finalist

A fundamental cornerstone of 24 Hour Home Care is our devotion to progress. This perpetual growth creates better solutions and an even higher bar for our outstanding, quality care. Recently, 24 Hour Home Care was named as a finalist for the Patrick Soon-Shiong Innovation Awards, presented by the Los Angeles Business Journal. The award acknowledges organizations that show innovation in the business industry in California. Our team is so humbled to be one of the 15 pioneering companies that was selected.

The Innovation Awards focused on one of our programs especially- our Hospital To Your Home™ Care Transitions Program, in which our staff provides systematic support to our clients within the first month after their hospital discharge. A research project presented by HealthCare Partners and 24 Hour Home Care on the link between professional caregiving and hospital readmissions has shown that our program has had a significant impact on decreasing hospital readmissions for patients diagnosed with congestive heart failure.

On November 18th, 2014, members of the 24 Team accepted our award at the Symposium and Awards Dinner. We are deeply grateful for this honor, and would like to thank the many local communities that have generously supported our work.
