Certified Fellow, Gavin Ward, Moderates ACHE Case Management Panel at 2015 National Readmissions Summit

24 Hour Home Care served as an exhibitor at the National Readmissions Summit 2015 in Anaheim, CA, on April 16th. Dr. Eric Coleman, creator of the popular Care Transitions Intervention™, spoke on the best practices in readmission prevention and care transitions.  Our very own Gavin Ward, Regional Director of Strategy and Partnerships and the very first Certified Fellow in Readmission Prevention by the National Readmission Prevention, moderated a panel with thought leaders from Kaiser Permanente, St. Joseph Health System, MemorialCare, Adventist Health, and Hoag Hospital. Other organizations leading discussions at the event included leaders from The Centers for Medicare and Medicaid Services, Cedars Sinai, and the Hospital Association of Southern California.
