24 Hour Home Care Sponsors the 10th Annual Case Management Conference

The 10th Annual Case Management Conference, hosted by the American Case Management Association- Southern California Chapter, was held at the San Diego Convention Center on February 6th, 2014. As a sponsor and exhibitor, 24 Hour Home Care staff was on-hand to discuss innovative solutions to prevent hospital readmissions, as well as California Health Care Reform and its effect on home care providers. The conference provided attendees with Case Management education on a variety of topics, including effective communication between patients and staff, the role of Case Managers in emergency department settings, and the growing importance of social workers in health care.
