24 Hour Home Care attended the first annual Southern California Readmission Summit on February 6th, 2014 at the Bahia Corinthian Yacht Club in Corona Del Mar. Southern California’s leading medical institutions participated in the conference to discuss their readmission prevention programs and resulting outcomes. Key speakers from Cedars Sinai Health System, Torrance Memorial Health System, University of California- Irvine, and Quality Improvement Organization covered strategies to prevent hospital readmissions, such as medication management, follow-up from hospital personnel, and helping patients make connections with meal preparation services and home health providers. Over 230 professionals in the health care industry attended the conference, which was hosted by the California Readmission Prevention Collaborative.
