To provide the best service possible 24Hr HomeCare offers a free in-home consultation. A 24Hr HomeCare representative can meet with you or your loved one to develop a customized care plan tailored your specific needs.

Here is a breakdown of what is covered:

  • Customized Care Plan – we will set up a care plan based on your needs, which includes:
    • General Client Information (address, phone number, height, weight, date of birth etc.)
    • Diagnosis
    • General Instructions
    • Medications (type, dosage and frequency)
    • Hours of Service
    • In-depth coverage of daily duties (personal care, domestic duties, transportation, meal preparation etc.)
  • On-Call Emergency Guide – we set up all emergency information, which includes:
    • Primary Physician
    • Additional Physician
    • Pharmacy
    • Emergency Contacts
    • Advanced Directives (legal guardian, power of attorney, emergency instructions)
  • Client Health Information – we cover some important information about health history:
    • Health History
    • Vitamins/Supplements
    • Limitations (psychological/neurological impairments, precautions etc.)
    • Allergies
    • Surgeries
  • Caregiver Profile – we find out what type of Caregiver would be a good fit for you, by covering such items as:
    • Personality preference – outgoing/reserved
    • Pro-Active or good at taking orders
    • Experience
    • Driver
    • Personality and Interests

In addition, we answer any questions you or your love one may have, cover all of 24Hr HomeCare’s policies/procedures and, discuss our rates. Furthermore, we would have you sign a one page agreement if you decide to choose 24Hr HomeCare to provide services for your father (no commitment is necessary after the consultation).
